Sandie Olosky, 35, the Metro bus driver involved in the head-on collision in April that killed a man, will be fired by Metro.

The accident occurred on Highway 164 when Olosky failed to slow with traffic and then swerved into oncoming traffic to avoid hitting the vehicle in front of her. The bus hit the bumper of the vehicle in front of her and then collided with a truck driven by Michael Dahlquist, 21. Dahlquist died at the scene.

Metro has stated that the crash was a "severe, preventable accident"

Olosky may be prosecuted for second-degree negligent driving.

Claims against Metro for injuries involving bus accidents must be filed with King County and properly served on the Clerk of the County Council. "Tort claim forms" are required whenever there's a personal injury claim against a government entity. The procedural requirements associated with tort claim forms can be tricky and, if not complied with, may bar any claim. Experienced counsel should be retained to assist with such claims.
